I have added this to the list of enhancement requests.

2) Toolbar buttons that can have macro assigned to.

This is already possible. Go to Macro > Library, select your macro, check the "Toolbar button" option, and assign a bitmap. Now you can right-click on the toolbar to customize it and add the macro button.
