Preferences - Key Configuration - stopped working in update to V9

I just updated to version 9.0.2.1621 from version 8.0.4.
In my preferences setup I have defined under Key Configurations these values:
Session/Log on - Shift+F12
Session/Log off - Ctrl+F12.

The Shift+F12 log on still works but the Session/Log off no longer works.

Is this an issue with the new version 9 of PLSD?
Can I correct it in the preferences setup?

Version 9.0 supports multiple connections, so the Session > Log off menu now has a sub menu where you can select a connection. If you disable the "Allow multiple connection" option (Tools > Preferences > Oracle / Connection), this will be a single menu item again.
 
I also upgraded recently to 9.0.5.1648 from v8 (not sure of the version).

None of my custom shortcuts work, even if I try going back to the defaults and resetting them.

I'm trying to make Ctrl + O open a new SQL window and Ctrl + W close the current window.

This worked just fine in v8. Any tips?
