Suppose the connection to the DB is broken when a transaction is currently open. For example because there is a VPN problem.
Then a rollback is then correctly acknowledged with an error ora-03114. The icons still show an ongoing transaction.
After access to the network is possible again, a DB connection is created again. The icons still show an ongoing transaction. Even a new rollback does not result in a change. Only after the DB has been changed again does a rollback result in the corresponding icon being disabled.
