Unable to add buttons to toolbar version PL/SQL Developer 11.0.3.1770

After upgrading to PL/SQL Developer 11.0.3.1770 I cannot add items to personal toolbars. Has this changed? If I bring up customise I don't see the option to add to my toolbars.
My prior toolbars went away with the upgrade and I see that is a known issue.

To add your own toolbar:

• Right-click on the toolbar area
• Select "Customize"
• Go to the "Toolbars" tab page
• Press the "New" button

Now a new toolbar will appear in the toolbar area and you can subsequently drag&drop commands from the "Commands" tab page.

If this does not work for you, can you let me know where it fails?
