toolbar enhancements

would it be possible to have custom toolbar buttons? that way you could map a macro to the toolbar, or a menu command etc...

This is actually one of the new features in PL/SQL Developer 5.1. You can create custom toolbar buttons for Marcos, External Tools, and Documents.

To minimize mouse travel you can assign a function key to the menu items (Tools > Preferences > Key configuration).
