Go to Tools -> Preferences -> User Interface -> Toolbar.
I was setting up some new buttons and I wanted to include a new separator. When I was dragging it, I inadvertently released the mouse button while still over the "Available Buttons" list. I got a new Separator there and one cannot get rid of it.
This also happens with "New Bar".
