Removing a shortcut by right-clicking and choosing "Remove from Quick Access Toolbar" doesn't get remembered when closing the program.
Testcase:
1. Start the program.
2. Right-click the Quick Access Toolbar and add a new shortcut.
3. Close and re-start the program. Note: the shortcut is now there.
4. Right-click the shortcut and choose Remove from Quick Access Toolbar
5. Close and re-start the program. Note: the shortcut is STILL there.
6. Right-click the shortcut and choose More Commands... and drag the shortcut off of the Quick Access Toolbar.
7. Close and re-start the program. Note: the shortcut is now gone.
