quick access toolbar

Hi, after upgrade to Version 12.0.4.1826 (64 bit)
disappeared "Quick Access Toolbar"
How to fix this ?

I have yet to upgrade to 12.04 but when I upgraded from 12.02 to 12.03 I lost all the icons that I added to the quick access toolbar.

Is it possible to save these shortcuts upon upgrade.

Toolbars are not modified during an upgrade.

Note that the Quick Access Toolbar will be replaced by regular toolbar when switching from a Ribbon system to a Menu system.
